Accreditation

This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of the Cleveland Clinic Foundation Center for Continuing Education and the Society for Heart-Brain Medicine. The Cleveland Clinic Foundation Center for Continuing Education is accredited by the ACCME to provide continuing medical education for physicians.

The Cleveland Clinic Foundation Center for Continuing Education designates this educational activity for a maximum of
11.75 AMA PRA Category 1 Credits^TM. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Participants claiming CME credit from this activity may submit the credit hours to the American Osteopathic Association Council on Continuing Medical Education for Category 2 credit.

The Ohio Board of Nursing will accept at face value the number of contact hours awarded for an educational activity that has been approved for CE by a nationally accredited system of CE approval. Therefore, the 11.75 AMA PRA Category 1 Credits^TM will be accepted for nursing credits.

Valet parking is available at the Sheraton Chicago Hotel & Towers for $48 (12-24 hrs).  As an alternative, there are several self-parking garages located near the hotel.  Ogden Plaza Self-Parking (connected to the hotel via an enclosed walkway) has a maximum 24 hr. rate of $34.
