Pre-registrations are accepted by fax or online
until 4:00 pm EST December 4, 2008.
To register after this date, you must do so
on-site.
In case of cancellation, a full refund
will be made if canceled by November 11,
2008. After November 11, 2008, a $150
cancellation fee will be deducted from
your refund. Written notification of your
cancellation is required in order to process
your refund.
No refunds will be issued after
December 31, 2008.

Registration
This course uses our online registration system which instantly approves credit card transactions and issues you a receipt immediately.
Instructions for first time users:

Login initially by selecting a registration category (Physician, Nurses, etc) from the drop down menu.

Provide your personal information (name, address, telephone, and a 4 digit number password of your choice). The password is required and will serve as your login identification for all future registrations.
- Select course options and enter payment information in the next screen.
Registration fee includes syllabus, lunches, refreshment breaks, breakfast symposium, evening symposium and banquet.
