Pre-registrations are accepted by fax or online
until 5:00 pm ET, November 28, 2008. To
register after this date, you must do so on-site.
In
case of cancellation, a $50 cancellation fee will
be deducted from your refund if cancelled after
November 28, 2008. Written notification of
your cancellation is required in order to process
your refund.
NO REFUNDS WILL BE ISSUED
AFTER JANUARY 31, 2009.

Registration
This course uses our online registration system which instantly approves credit card transactions and issues you a receipt immediately.
Instructions for first time users:

Login initially by selecting a registration category (Physician, Nurses, etc) from the drop down menu.

Provide your personal information (name, address, telephone, and a 4 digit number password of your choice). The password is required and will serve as your login identification for all future registrations.
- Select course options and enter payment information in the next screen.
Registration fee includes syllabus, continental breakfast, refreshment breaks, welcome reception, & optional
