Description

Chronic disease management involves diagnosis, evaluation, and treatment, but also depends greatly on a patient’s ability to practice self-care. At the Cleveland Clinic, we recognize the need to help our patients help themselves. This one-day summit brings together experts from medicine, case management, nursing, social work, psychology, addiction, and more to achieve what the title suggests: promote growth and change in the most complex, challenging and difficult of patient populations. The summit will feature keynote addresses, didactic sessions, break-out skills-based workshops, and educational resources, while offering continuing education credits for physicians, nurses, social workers, and other clinicians.

The registration deadline is Tuesday, April 18, 2017 at 5:00 pm EST. To register after April 18 please contact Shana Spade at 216.444.9380 or spades@ccf.org for details on how to register onsite.

Cancellations will be accepted until April 14, 2017. In case of cancellation, a $50 cancellation fee will be deducted from your refund. Written notification of your cancellation is required in order to process your refund. NO REFUNDS WILL BE ISSUED AFTER APRIL 14, 2017.

Registration fee includes continental breakfast, lunch, refreshment breaks and a copy of the book Motivational Interviewing: A Guide for Medical Trainees, edited by Antoine Douaihy, a $50 value.
