Registration fees for this in-person program include:

*Letter from program director is required to receive the discounted fee. If letter is not received two weeks prior to the activity, the full physician fee will be charged.

REGISTRATION AND CANCELLATION

In case of cancellation, email notification is required. A full refund will be provided if your cancellation request is received by 4/24/26. After 4/24/26, a $50 cancellation fee will be deducted from your refund (except for Non-physician, Resident/Fellow/ Trainee). There will be no refunds issued after 5/8/26.
